The mechanism
The chart is a loop of four stops: flat revenue, an easy guess that it's a sales problem, a wrong fix like hiring a salesperson, and six to twelve months passing. Then the arrow lands back on flat revenue. The one arrow out of the loop, at the top, points to actually finding the cause first. Nothing inside the circle ever tells you what's really wrong, so each lap just costs another year of attention.
